Bud light UFC Flyaway Consumer Sweepstakes Giveaway

Must be 21 years or older and a Hawaii resident or active Military personnel living in the State of Hawaii. Void where prohibited.
1.) No purchase is necessary. To enter, snap a picture of the Quick Response Code and fill out all required information on the Enter To Win webpage, including name, age, address, email address, and store location. No mail-in entries will be accepted. Entries will be accepted from November 5th, 2012 – November 30th, 2012.. One entry per person per week will be accepted. For Drawing – entries must be received by 9/30/12. No mechanically reproduced or photocopied entries permitted. For an official entry form and a copy of the rules, send a self-addressed, stamped envelope to: Bud Light UFC Flyaway Consumer Sweepstakes, c/o Anheuser-Busch Sales of Hawaii, 99-877 Iwaena St, Aiea HI 96701. Limit one request per envelope. Requests must be received by November 20th, 2012. Neither Anheuser-Busch Sales of Hawaii (”Sponsor”) nor any of its agents are responsible for illegible, lost, late, damaged, incomplete, postage-due or misdirected mail or entries. All materials submitted become the property of Anheuser-Busch Sales of Hawaii and none will be returned.
2.) There will be one (4) random drawings (two grand prize winners, one second place, one third place). Four (4) winners will be selected in a random drawing from among all eligible entries received by an Anheuser-Busch Sales of Hawaii representative, whose decisions are final in all matters relating to this game. Drawing dates: 12/3/12. All non-winning entries do not rollover and are not eligible for subsequent drawings. Winners will be contacted via phone.
3.) There will be Four (4) prize winner selected for the State of Hawaii. The approximate retail value of prize for the Bud Light UFC Flyaway is $2,000.00. Winners may not substitute or transfer prizes but Sponsor reserves the right to substitute prizes with prizes of equal or greater value. One prize awarded per family or household.
4.) Odds of winning depend upon number of entries received.
5.) Winner must be 21 or older at the time of December 3rd, 2012 and will be required to complete, sign and return an affidavit of eligibility and liability release within 72 hours of prize notification. In the event of non-compliance within this time period, prize will be forfeited and an alternate winner will be selected. Any prize notification or prize returned to Sponsor or its agencies as undeliverable will result in disqualification and the awarding of that prize to an alternate winner.
6.) Sweepstakes is open to all State of Hawaii residents and active military personnel living in the State of Hawaii who are 21 and older, excluding employees and their immediate families of Anheuser-Busch Inc. and its affiliates, subsidiaries, advertising and promotion agencies, wholesale distributors and retail licensees. Proof of residence required. The contest is void where prohibited by law and subject to federal, state, and local regulations. Taxes on prizes, if any, are the responsibility of individual winners.
7.) Acceptance of prize offered constitutes permission to use winner’s name, biographical information, and/or likeness for the purpose of advertising & promotion without further compensation as permitted by law.
8.) By accepting a prize, each winner agrees to release and hold Sponsor harmless from any and all losses, damages, rights, claims and actions of any kind resulting from acceptance, possession or use of any prize including without limitation, personal injuries, death, and property damage. By participating in sweepstakes, participants agree to be bound by all the Official Rules of this sweepstakes.
